Do not submit health information
The contact form opens an email draft. Do not include diagnoses, medical record numbers, insurance numbers, prescriptions, or other sensitive personal health information.
Before adding analytics or forms
Publish a complete privacy notice describing vendors, cookies, retention, data sharing, security, user choices, and applicable legal rights. Configure consent and data minimization before collecting information.

A general health information website is not automatically a HIPAA-covered entity. Legal obligations depend on the organization, services, relationships, location, and data practices. Obtain qualified legal advice.
